This includes providing leadership for planning, management,Implementation, integration and coordination of Case Management in support ofFacility goals dedicated to excellence, effective leadership, and financialStability.

Supporting the delivery ofThe high quality, cost effective patient care through activities based on facilityStrategic goals and objectives.ESSENTIAL FUNCTIONS:UnderThe guidance of the Director, the Manager is to assist in the planning andCoordination of Case Management activities, operational efficiency andEffectiveness as well as major impacts on all other hospital areas.AssistThe Director in providing direction to the staff of the Case Management teamAnd assists in reviewing and evaluating performance of the departmentStaff.

Involved in counseling andAppropriate disciplinary actions of staff members as requested.

AssistDepartment staff in meeting and adhering to all applicable federal, state,JCAHO, and local regulatory agency requirements and assist in the success ofThe department and facilities in any regulatory survey.

MakesDecisions in absence of the director to maintain acceptable operations based onStrategic goals and policies.

DisplaysIngenuity and foresight in determining the most appropriate solution in theAbsence of established guidelines.ConfersWith other department leaders when necessary to resolve proceduralDifficulties, clarifying department responsibilities, objectives and resolvingIdentified problems.

AdheresTo hospital/departmental attendance policy, work hours and maintenance ofPersonal appearance as stated in facility policy.MakesSuggestions on development of new system processes as needed to support newRegulations, software installation, changing environment, and needs of team.WorksWith the Case Management Director to monitor staff caseload, reorganizingDepartment as needed to support patient population on each unit.ApprovesStaffing schedule ensuring appropriate coverage to support patient caseManagement needs.MonitorsDaily reports of observation patients.

Understands financial implications ofLength of Stay requirements for organization and serves to improve efficiencyWhile ensuring safe, appropriate discharge planning throughout theOrganization.CollaborativelyWorks with the Case Management team and effectively engages employees,Maintains positive teamwork relationships, and strives to improve employeeEngagement.Develops,Coordinates and monitors strategies for obtaining, organizing and allocatingResources required for care transitions.AssistsTeam members in problem-solving complex patient situations.EstablishesCollaborative practice with nursing personnel, physicians, and health careProfessionals regarding the plan of care and identifies variance in planImplementation.

IdentifiesNecessary resources or eliminates unnecessary resources needed to achieve qualityPatient outcomes as well as advocates for patients facing barriers toResources.

This includes identifying alternative options if needed resourcesAre not accessible.
